For your convenience, you may download and fill out our registration form by clicking on the "Our Practice" button and then bring the completed form in with you at your appointment.
Financial/Insurance
We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits. Although we do not participate in any HMO programs, we do participate in many PPO's and standard insurance. If you provide us with a carrier name, employer name and group number, we can even verify benefits before your first appointment. We will do everything we can to help you afford the treatment you need and want. For patients who require major work, payment plans, including CareCredit, are available. We also accept Visa, and Mastercard.
Cancellation
If you are unable to keep an appointment, we ask that you provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to other patients, many seeking urgent care. A fee may be charged if a patient does not show up for an appointment without sufficient notice.
